Cottonwood Clinic is a family-orientated, patient centered clinic offering primary care for patients of all ages.

The clinic was established in 2104 by Heide Applegate, FNP-C allowing her to pursue the desire to provide high quality medical care in a patient centered clinic in her hometown.

We pride ourselves in providing compassionate and personalized care for our patients and their families.

Our Providers:
Heide Applegate, FNP-C
Heide is well known for her commitment to providing high quality compassionate care to both children and adults in the area. Prior to opening Cottonwood Clinic in 2014, she practiced at Pintler Family Medicine and Community Hospital of Anaconda. She has been in the healthcare field for 25 years and a nurse practitioner for the past 15 of those. She lives in Deer Lodge with her husband Mike and their children on the family ranch.
Heide considers herself blessed to be able to practice in the Deer Lodge and hopes to care for you and your family for many years to come.
Caring for her patients is her passion, and it shows.

Dan Curran, FNP-C
We are excited to announce that Dan Curran, FNP has joined our clinic.
He will be seeing patients for acute/sick visits as well as new patient establishments.

Our Staff:
Toni Flack
Toni is our friendly face and voice of the front office. She joined our practice in 2015 and we have been thrilled to have her. Born and raised in Montana, she resides in her hometown of Anaconda with her husband Steve.
Toni is detail orientated and responsible for our day to day operations. She is always happy to lend a helping hand.
Shelly Cunneen
Shelly is our part time receptionist who was planning to retire until we talked her in to coming to work for us. She has many years working in the health care field in medical records and front office. Her and her husband Bill reside in her hometown of Butte where they have raised 4 children and enjoy spending time with their grandkids.
Andine Woodruff, LPN
Andine is our clinic nurse and she is as unique as her name. She has been a nurse for over 30 years in a variety of settings including home health, long-term care, and clinic nursing. Andine is known for her kind and caring demeanor. She has been working together with Heide for the past 8 years providing excellent care to our patients.
Kacie Boggess, RN
Kacie joined our team in 2022 and we couldn't be happier to have her! She was previously a school nurse and worked in acute care in the hospital setting which she continues to do on weekends to keep her acute care skills up to date.
She is a local and her husband and her reside in Deer Lodge along with their 2 sons and a variety of animals including cows, goats, and dogs to name a few! l
